The Montgomery County Department of Health and Human Services Office of Public Health is conducting its first Community Health Assessment (CHA). The assessment will help better understand the community’s health status, concerns, and priorities. 

The data collected will be used to develop a collaborative, community-wide, transparent, and long-term Community Health Improvement Plan that will be shared with the community. 

But your help is needed in two ways: 

• Take the survey! The County would love to hear from you! To take the confidential survey, get notified when the report is available, and for more information, visit www.montgomerycountypa.gov/CHA. The survey is available in several languages. 
• Spread the word! If you have family or friends who live in Montgomery County, please send them the link and encourage them to take the survey. The CHA webpage contains flyers, as well as graphics and text that can be used for social media posts, newsletters, and emails. Please use these resources to share information about the CHA with others who live in Montgomery County! If you are part of an organization in Montgomery County or have family or friends who live in Montgomery County, please send them the link and encourage them to take the survey.
