Important information regarding second dose appointments for Covid-19 Vaccine on WEDNESDAY, 6/16/21.
This is a Reminder - If you are scheduled for a SECOND DOSE of Pfizer Vaccine on WEDNESDAY, 6/16 at the GIANT FOOD STORE - 

Location:  UPSTAIRS COMMUNITY CENTER ROOMS located at 315 YORK ROAD, WILLOW GROVE, PA. 

Appointment times for Dose 2 on WEDNESDAY 6/16 are the same as the appointment times from Dose 1 on given on WEDNESDAY, 5/26. 

To be fully protected against the Covid-19 virus, it is very important that you receive your second immunization within 42 days of your first immunization.  

NOTE: You will need to complete ANOTHER Informed Consent form for this appointment. The informed consent form must be filled out prior to EACH immunization.   We will have copies available the day of your immunization for you to complete if you did not do so before your appointment. 

Please also bring your white CDC Covid Vaccine Vaccination record card with you to this appointment.  Any patient under the age of 18 MUST have a parent or guardian present.
